Report a Concern

Individuals may submit accommodation or access concerns. Upon receipt, the ADA/504 Coordinator or designee will conduct a timely and impartial review of concerns.

Submit a report of discrimination, harassment or workplace violence through the Equal Opportunity incident report form.

Reports can be submitted online using the applicable online form, or by phone, in person, or by mail.

Any person who is informed of a crime or safety concern should document the information and submit a report to the App State Police as soon as possible.
